Let's start with the most officially proven medical use of CBD. Earlier this year, the FDA approved the first-ever drug containing CBD, Epidiolex, to treat two rare forms of pediatric epilepsy. To get to that point, the drug's manufacturers had to do a whole lot of randomized, placebo-controlled trials on humans. They had to study how much children could take, what would happen in case of overdose, and any possible side effects that would occur.

Medical research can and is being done with schedule 1 substances, including CBD and other active ingredients in marijuana, but there are strict regulations and administrative hurdles associated with this status. According to the Federation of American Societies for Experimental Biology, the DEA is currently conducting a scientific review of CBD to elucidate its pharmacology and abuse liability and to identify gaps in the published literature. Later in 2014, researchers reported on preliminary results of a study involving children with treatment-resistant epilepsies in an expanded access “compassionate use program.” Patients received a purified 98 percent oil-based CBD extract called Epidiolex, which is made by GW Pharmaceuticals. After 3 months of treatment, 39 percent of the 23 patients had more than a 50 percent reduction in seizures, with a 32 percent median reduction. These preliminary results support the animal studies and survey reports that CBD may be a promising treatment for treatment-resistant epilepsy and it is generally well-tolerated in doses up to 25 milligrams per kilogram of body weight. Genetic manipulations that impede CB1R activation are anxiogenic [35], and individuals with eCB system gene polymorphisms that reduce eCB tone—for example, FAAH gene polymorphisms—exhibit physiological, psychological, and neuroimaging features consistent with impaired fear regulation [36]. Reduction of AEA–CB1R signaling in the amygdala mediates the anxiogenic effects of corticotropin-releasing hormone [37], and CB1R activation is essential to negative feedback of the neuroendocrine stress response, and protects against the adverse effects of chronic stress [38, 39]. Finally, chronic stress impairs eCB signaling in the hippocampus and amygdala, leading to anxiety [40, 41], and people with PTSD show elevated CB1R availability and reduced peripheral AEA, suggestive of reduced eCB tone [42].
Inducible nitric oxide synthase (iNOS) – CBD reduced the overexpression of iNOS in response to colitis. iNOS overexpression is well correlated with disease activity with colitis, and inhibitors of iNOS lead to improvement in experimental models of IBD. iNOS results in high-output production of NO, which results in oxidative damage to the intestine via reactive oxygen species (ROS).

In the meantime, some physicians are forging ahead — and cashing in. Joe Cohen is a doctor at Holos Health, a medical marijuana clinic in Boulder. I asked him what CBD is good for, and he read me a long list of conditions: pain, inflammation, nausea, vomiting, intestinal cramping, anxiety, psychosis, muscle spasms, hyperactive immune systems, nervous system degeneration, elevated blood sugar and more. It’s true that a preliminary study found hints that cannabinoids might reduce beta amyloid proteins in human brain cells, but the study was done in cells grown in a lab, not in people. As for cancer, the FDA sent warning letters last year to four companies that were selling products that claimed to “prevent, diagnose, treat or cure” cancer.
